If this is your first time booking with us, once you book your first appointment, you will receive an email confirmation.

Please open the email message, click on the link that says “FILL OUT INTAKE FORM” and it will take you through any required intake forms. Please fill them out completely, providing all information requested, answering all questions as best you can. Continue through all sections of the intake forms until you can successfully click on the button that says “Submit Intake Form”. When you read “Thanks, we’ve received your info.” on the next screen, your registration is complete. You must complete your intake forms within 24 hours of booking your first appointment, and before you arrive to the clinic.

You must provide a credit card on file, to be securely stored with encryption, which you will be prompted for as you complete your booking. This fulfills your obligation and agreement to the cancellation policy, which requires a minimum of 24 hours’ notice before the scheduled appointment time if cancelling. If you cancel inside of 24 hours, or do not show to your appointment, the practitioner reserves the right to charge you the full amount of the service booked unless the time is filled by another person.
